You should address the reception and fill the request form (F 025-025-1/a) if you wish to be registered in „Aušros Medicinos Centras“. Submission of personal identification document is mandatory (passport, personal identity card, child‘s birth certificate). The request form is filled with computer at the reception if a person is under-aged (0-18 years old), illiterate or otherwise in the need of help. The request of under-aged person is signed by one of the parents or foster-parents who need to indicate their names and surnames; in case of illiteracy the same procedure is done by two employees of the institution, who also indicate their names and surnames.
When registering with a doctor patients must present the following documents:
-Children up to 16 years old must present birth certificate and the ones between 16 and 18 years old must present either passport or personal identity card.
-Employed people must present either passport or personal identity card and the card of health insurance or social insurance.
-Unemployed people must present either passport or personal identity card and the document from employment exchange.
-Pensioners must present either passport or personal identity card and the document of the pensioner.
-Disabled people must present either passport or personal identity card and the document of working level.
-Unemployed full-time students and high school students who are above 18 years old must present either passport or personal identity card and the card of compulsory health insurance.
-Full-time students of the European union must present either passport or personal identity card and the card of compulsory health insurance.
Additional information:
-Services are provided starting the same day when the request form is filled.
-When replacing health care institutions, you do not need to inform the previous one.
